OFFICE POLICIES
AFTER HOURS AND EMERGENCIES

If you are experiencing an emergency, dial 911 or go to the nearest emergency room. For urgent concerns after hours, an on-call triage service is available. The doctor on-call will be notified and return your call. If your call is non-urgent, please call the office during our business hours to speak with one of our nurses.

NURSING LINE

Please contact our nursing line during normal business hours if you have questions or medical concerns that cannot wait until your next appointment. The nurses will evaluate your situation and determine if you need to see one of our providers as well as assist with arranging that visit. If appropriate, the nurses will forward questions directly to your provider or another provider in the practice.

PRESCRIPTION REFILLS

If you need a refill before your next visit, please request the refills with our nursing line or through the online patient portal. Please make the request several days before you need the refill to allow ample time to process the request. Please have your prescription information ready (ie medication name, dose, preferred pharmacy, etc). Your request may be denied if you are overdue for a routine examination, have not been evaluated by the prescriber in over one year, or if the medication is deemed not appropriate to refill. For new prescriptions, you will need to meet with your provider. 

TEST RESULTS

You will not receive notification of results that are considered "normal". Test results can take 1-3 weeks to return, depending upon the type of test. Results may be released onto the patient portal at the discretion of the provider. If you are due back in the office shortly after completing the lab work, the provider will review the results with you then. If a result is considered "abnormal", you will be notified of the result along with any additional testing, treatment, or follow-up recommendations as determined by your provider. In certain situations, we may suggest you schedule an office visit to discuss your results.

REFERRALS AND MEDICAL RECORDS

If you have an appointment (office visit or ultrasound) that requires a referral, please be sure to obtain it well before your appointment. Our receptionists will inform you if a referral is necessary when you book the appointment. In addition, if you are following up with our office about a medical concern, please be sure to bring any and all pertinent medical records with you to your appointment. Alternatively, you may have your medical records faxed to our office.

 

Please call our office prior to your scheduled appointment to make sure that your referrals and/or medical records were received. If necessary referrals or medical records are not on file the day of your appointment, we may need to reschedule your visit.

You may have your medical records transferred to another practice, provider, or facility at any time. Please request your records in person or over the phone. You will need to sign a medical release form and provide the name, address, phone number, and fax number for your new provider or practice. A $20 fee will be charged if you request a printed copy of your medical records. This fee covers the cost of ink, paper, and printer maintenance. Please be sure to request your medical records in advance to ensure that your transition of care is seamless. 

LATE AND MISSED APPOINTMENTS

We value and respect your time at Andover OB/GYN. One of the ways we are able to keep appointments running on schedule is when patient's arrive on time. We kindly request that you arrive 5 to 10 minutes prior to you scheduled appointment time in order to complete the check-in process and any necessary paperwork. You may complete these forms ahead of time by downloading and printing them from this website (see the links below). 

 

We also understand that life happens! If you are aware that you will be late, please call our office so that we may attempt to accommodate you. If you are late and we are unable to see you the day of your appointment, we will assist with rescheduling the visit to the next available day. 

Missed appointments decrease access to appointments for other patients. At Andover OB/GYN, we attempt to accommodate urgent-same day visits. In order to minimize the number of lost appointments and improve access to visits for our patients, we have adopted a "No-Show" Policy. A no show is defined as a missed appointment without giving notice or giving less than 24 hours notice.

 

No Show Policy

  • A "No Show Fee" in the amount of $25.00 will be charged to the patient for each missed appointment

  • If you have 3 or more consecutive no shows, you will need to speak with the office manager before you can reschedule the appointment again

CANCELLATION POLICY

Your health is our top priority; attending scheduled visits will minimize health complications. We strive to improve patient outcomes and access to visits with our cancellation policy. As stated above, we request at least 24 hours notice if you cannot attend your appointment. Failure to do so will result in a "No Show Fee" being applied to your account. You will not be charged a "No Show Fee" if you cancel or reschedule your appointment with at least 24 hours notice. We appreciate your cooperation.

Please download and complete all necessary forms prior to your visit. Don't forget to bring these completed forms with you to your appointment. This will expedite the check-in process and assist with keeping appointment on schedule.
